Your care is recorded on a digital care plan. Your care plan will be reviewed regularly and any changes discussed with the care team making sure changes needs are met.
Our staff will respect your values, take into account your wishes, preferences and expressed needs. We will work with you to make sure there is good communication, ensuring you are comfortable, safe, provided with emotional support and have access to good care and treatment when you need it.
We believe by putting you and your family at the center of decision making and working alongside staff you will get the best outcome. We will meet with you regularly for reviews and send you questionnaires to obtain feedback about your satisfaction with the care and treatment we provide.
